Athletic Physical/Registration Forms

Please use the attached forms to register for an athletic team.  When complete, please bring required forms to the nurses office.

The New Jersey Department of Education requires that athletic physicals be conducted in the “medical home” of the student. This means in the office of the student’s personal physician, at the parent’s expense. Only in cases where a student-athlete does not have a “medical home”, or some other emergent situation exists, is the school physician permitted to perform the examination in his/her office.
